94 Buick LaSabre-Serpintine belt comes off several times?

94 Buick LaSabre. In August the serpentine belt slipped off & Voltage light lit. Car very
hard to steer. I barely got to a nearby repair shop. Mechanic said the belt was not
broken but we both thought it was no doubt old & appeared worn. He replaced the belt. That
was summer. Now 4 months later and very cold weather (20 degrees) The belt has come off
twice in one week..What are possible causes?
